Carebridge is a 501(c)(3) non-profit organization committed to ensuring all Michigan communities have access to high-quality, affordable primary care.

Our mission is to strengthen the primary care network by helping independent practices sustain, strengthen, and thrive. By supporting private practices, we ensure that families have consistent access to the care they need—whether it’s preventive services, chronic disease management, or timely specialist referrals.

We provide expert consulting and tailored support services to private practice primary care physicians (PPPCPs), empowering them to overcome financial and operational challenges so they can continue to serve their communities.

Our History

Carebridge, founded in 2011 as a 501(c)(3) non-profit organization, was created to facilitate participation from Michigan’s Physician Organizations (POs) in the Michigan Primary Care Transformation (MiPCT) Project.

MiPCT was one of the earliest, largest, and longest-running multi-payer initiatives in the United States to showcase the benefits of care management in Patient-Centered Medical Homes (PCMHs). The project aimed to enhance care coordination, improve healthcare quality, and reduce costs by integrating care managers into primary care practices. These care managers were instrumental in delivering personalized care, managing chronic conditions, and addressing patients’ social and behavioral health needs.

As the largest participating MiPCT entity, Carebridge represented 80 practices across 8 physician organizations, playing a pivotal role in engaging practices and POs, optimizing performance, and achieving population-level improvements.

With the conclusion of federal funding for this initiative, the Carebridge Board of Directors has shifted the organization’s focus toward ensuring the sustainability of private practice primary care physicians (PPPCPs).  Today, Carebridge is dedicated to providing services that support smooth leadership transitions, safeguard operational stability, and ensure that practices continue to deliver high-quality care to their communities.
